Well, this is a serious problem. There may be two reasons for these two situations in online search.
1. There may be a problem with the disk IO (through logging, monitoring, troubleshooting, etc., it is sure enough that it is the problem of IO)
2. There may be a physical failure of the disk (call the computer room, and there is no problem with the disk (raid5). But that doesn't mean it doesn't exist.
