Surgical resection is the first choice for the treatment of liver cancer at present, but 70-80% of liver cancer patients have been diagnosed with tumor dissemination and metastasis, which is in the middle and late stages of liver cancer and loses the opportunity of surgery; even if surgical resection is performed, the recurrence rate of metastasis after 5 years of liver cancer is as high as 60-70%.
Source: metastasis and recurrence of wkhs liver cancer is the main reason affecting long-term survival of patients, and has become the bottleneck to further improve the overall efficacy of liver cancer.
At present, the panoramic map of gene mutation and molecular expression of metastatic liver cancer is still extremely lacking, and the evolutionary rules of clonal selection in the process of metastasis of liver cancer are still unclear, which leads to the lack of sufficient theoretical guidance for clinical diagnosis and treatment of metastatic liver cancer.
Academician Fan Jia's team integrated and analyzed multi-omics data, systematically characterized the panoramic atlas of spatio-temporal evolution of liver cancer metastasis for the first time in the world, revealed the complex evolutionary trajectory and clonal selection mechanism of liver cancer metastasis, and provided valuable data and theoretical basis for the development of new markers and new targets for liver cancer metastasis prediction.
In this study, 182 patients with hepatocellular carcinoma were screened from more than 20,000 liver cancer patients in Zhongshan Hospital Affiliated to Fudan University and Tianjin Cancer Hospital, and retrospective multi-regional sampling of surgical resection samples of primary lesions, metastatic lesions and intrahepatic recurrent lesions was carried out. Finally, 461 paraffin-embedded tissues were collected for multi-group sequencing analysis such as genome, transcriptome, single cell transcriptome, spatial transcriptome and digital pathology. It provides a wide range of spatiotemporal multi-group data for the study of heterogeneity of liver cancer metastasis.
The research team further studied the clonal pattern of metastatic colonization for metastatic samples with multi-region sampling and sequencing results, and found that about 3/4 of metastatic tumors were monoclonal colonization and the remaining 1/4 were polyclonal colonization.
Compared with monoclonal metastasis, patients with polyclonal metastasis have faster disease progression and worse prognosis. Further molecular mechanism studies suggest that hypoxia signaling pathway activation may be a key driver of polyclonal metastasis.
